As to some sort of answer to your mystery question, if what you have
described was indeed a motion picture, or TV show scenario, consider that it
might be reasonable to assume the production team thought that they might get
98% of those viewing your scene to suspend belief for the duration of the
show, and not question the feasibility of what they had witnessed.
Unfortunately it seems that you managed to find a continuity error because
some studio prop person failed to ensure a prop camera was turned on.
